Client Forms


We will request a credit card upon booking. Should you need to cancel or reschedule services we kindly ask for a 24-hour notice. For cancellation on the day of service, or a no-show to your appointment, you will be responsible for a fee of $50.  

Clients who repeatedly cancel at the last minute or do not show for their appointments will be expected to put down a $100 non-refundable deposit. That deposit will be applied to the client’s appointment at the time of service.

Treatments such as SmoothGlo, Ultherapy, Genius, C02, and Coolsculpting will require a 10% non-refundable deposit at time of booking and will be applied at the time of service.

Gift Certificate Policy

Gift Certificates may be redeemed for services or products. They are non-transferable, and cannot be redeemed for cash. Gift Certificates must be utilized by the expiration date.


Initial consultations are at no charge so that together we can determine the best skin care path for the client.  Multiple consultations will be charged $50.

Security Measures

To safeguard your personal information, we have implemented secure page logins and locked user accounts to email addresses so that any attempt to access login information cannot be sent to non-authorized parties.
